2 QPU w Instructions and Burden Statement

Quarterly Progress Update (QPU) form and Instructions.
All information is pre-populated into the HRSA Electronic Handbook (EHB) from existing information collected during the application phase.
When prompted, grantees will mark each activity with one of 5 choices: Activity is on Schedule, Activity is Complete, Timing is off Track, Activity
will be missed if action is not taken, and Activity cannot be achieved. Grantees may write any comment, such as “complete”, in order to submit
the form.
