The Childcare Survey of Activity and Wellness (C-SAW) Pilot Study

The Childcare Survey of Activity and Wellness is being conducted on a random sample
of childcare centers regarding the policies and practices they have in place to support
children's diet and physical activity. Questions will be answer by a center director or
administrator on behalf of the center. The proposed information collection only
requests information about the childcare center and not the person. No confidential
business information is being collected.
The contractor will have the name, address, phone number, and e-mail address of the
childcare center in order to contact them for this study. This information will not be
provided to CDC and will not be linked to the final dataset sent to CDC. The
information will be destroyed at the end of the study by the contractor.
No PII or IIF is being collected. The Privacy Act does not apply.
