Hello,


Thank you for being part of the National Health and Nutrition Examination Survey, or NHANES! NHANES is an important national study run by the Centers for Disease Control and Prevention. Now that you are in the study, we want to tell you more about what to expect.


Our health representative will call soon to ask a few questions about your health. Then you will visit our mobile health center for a safe, free health exam.


We will give you the results of your exam. You can share them with your doctor. We will reimburse you for childcare, eldercare, and transportation expenses, or we can help you get to the health center. We will also give you a token of our thanks for your time.


During your telephone interview, we will go over the information in this booklet. It includes:


  • Information you will need for your telephone interview

  • A flyer describing your rights as a participant and all that we do to keep your information safe

  • Forms that ask for your permission to be part of the study

  • A list of the measurements we will take and the lab tests we will do

  • An envelope with a VISA© debit card inside. We will add the token of our thanks to this card, so please have it with you during the interview.
