Federal Post Card Application - Approval
Thank you for completing a review of Social Security Number (SSN) use within this
form. This office has reviewed and approved your request to include the SSN on the abovereferenced form for the following Acceptable Use: (13) Other Cases. The reference number
for this action is listed at the top of this memorandum. Please maintain a copy of your
justification and this approval in your records.
SSN use is approved indefinitely, until the form is superseded or rescinded or until all
States and territories require a reasonable alternative identifier instead of the SSN or truncated
SSN to register to vote or to obtain an absentee ballot.
