Thank you for participating in the National Youth Tobacco Survey, conducted by the Centers for Disease
Control and Prevention—or CDC. Your input today helps to understand the attitudes, knowledge, and
behaviors of teens across the nation.
This survey covers tobacco- and nicotine-related topics. You will be asked about cigarettes and electronic
cigarettes, cigars—including little cigars and cigarillos—smokeless tobacco, including chewing tobacco,
snuff, or dip; snus, and dissolvable tobacco products. You will also be asked about hookahs, pipe tobacco,
bidis, roll-your-own cigarettes, heated tobacco products, and nicotine pouches.
Remember, this is not a test about you or your school, so if you don’t feel comfortable answering a question—
you can skip it. Your participation is voluntary and does not affect your grade in this class—whether you
answer the questions or not.
This survey is anonymous. The five-digit code you were given to access the survey cannot be tied back to
you and should not be shared with others. You will not be asked to provide your name, and survey results
will never be reported by name, class, or school—so no one will know how you answer the questions.
Please complete the survey just one time during the class period it was assigned. It should take you about
20 to 35 minutes.
Here’s how it works:

Each section of the survey has a short introduction. Review them carefully, then read each question and
select your answer based on what you believe, know, or do.
Use the “Go Back” and “Continue” buttons at the bottom of the screen to navigate through the survey.
DO NOT use the back button on your browser.
Do the survey in one sitting. If you need to stop and log out for any reason, your access code will be
shown on another screen. Write it down, so you can log back in and continue the survey where you left
off.
Your answers will be saved automatically as you advance through the survey. When you reach the end,
you can review your responses to make sure you haven’t skipped any questions you wanted to answer.
When you are finished, select the “Submit” button and close your browser. And that’s it, you’re done!

Teacher Pre-Survey Script—National Youth Tobacco Survey
Thank you for your class’s participation in the National Youth Tobacco Survey, conducted by the Centers for
Disease Control and Prevention—or CDC. Your students’ feedback will be used to design educational programs
and other strategies to reduce tobacco product use among teens.
The support and encouragement you give students helps to ensure a high rate of participation, which is essential
in understanding the attitudes, knowledge, and behaviors of teens across the nation.
The survey should take students about 20 to 35 minutes and should be completed as part of students’ regularly
assigned classwork.
Here’s how you can prepare:

Review the Summary of School Arrangements form. It includes the planned date for students to complete the
survey and information about the parental permission form process.
Distribute a parental permission form to each student in your selected class 3 to 10 days before the survey
date. Depending on your instructional model, some students may receive a paper permission form to take
home. Students in distance learning can be sent an electronic version. If you need additional forms, please
reach out to your school contact person, listed on the Summary of School Arrangements form.
As students return their permission forms, it is important to keep track of those that are returned with a parent
refusal, so you can easily determine which students are eligible to participate. If your school is using an Optout permission form, a signed form will be returned only if parents do not want their child to participate. If
your school is using an Active permission form, only students whose parents returned the form marked YES
can take the survey. Do not provide the survey link to students who do not have permission to participate.
The type of permission form your school is using can be found on the Summary of School Arrangements
form.
On the survey date, or the day before, sign back into this website with your Teacher Code and watch the “Day
of Survey” video.
a. It will provide you with instructions on how to facilitate the survey administration in your selected
class.
b. And, it will prompt you to complete the Class Enrollment form. This is information we need to help
calculate a participation rate.
If you need assistance before, during, or after the survey administration, you can reach out to your assigned
Technical Assistance Provider, whose name and direct phone number are listed on the Summary of School
Arrangements form.
Survey results will be published next year, and you will be able to download the final report. For more information,

Teacher Day-of-Survey Script—National Youth Tobacco Survey
Welcome back and thank you for your class’s participation in the CDC’s National Youth Tobacco Survey. Now
that permission forms have been sent to all students in your selected class, here are a few easy steps you can
follow to give students access to the survey:
FIRST—Distribute the student access codes and survey link. Be sure to provide this information ONLY to students
with parental permission. If your school is using the Opt-out permission form, students whose parents returned a
form marked NO should not be given an access code and survey link. If your school is using an Active permission
form, only students whose parents returned a form marked YES should be given an access code and survey link.
Students can begin the survey by visiting nyts.cdc.gov from any internet-connected device. It should take about
20 to 35 minutes to complete.
Students should take the survey independently, and there should be no discussions about survey content. If
anything is unclear to students, remind them to do the best they can. They can skip any question they are
uncomfortable answering.
If students have any questions about topics raised in the survey after they’ve completed it, please refer them to
your school’s contact person, a school nurse, guidance counselor, or other appropriate resource.
NEXT—You’ll need to provide information about your class enrollment and survey participation. It is very
important that you complete the Class Enrollment Form on the day you distribute the access codes and survey
URL. It should only take two to three minutes.
Click “START” at the bottom of this page to begin the form. You will be asked to:

Enter the number of students on your official class roster.
Submit the number of parent refusals you received, if any.
Provide the number of students who:
o Are absent on the day the survey is planned.
o Could not complete the survey independently.
o Dropped class or school.
o Were expelled or suspended.
o Or have an extended absence.
And, share any other comments about your experience facilitating the survey.
If you need to log out before you’ve fully completed the form, simply use your teacher access code to log back
in. Once you’ve answered all questions on the form, review your responses and select “Continue.” Then, press
“LOGOUT” to save your responses and exit the form.
FINALLY—Follow up with any eligible students who were not present during the survey administration. Provide
them with the URL and an access code, and encourage them to complete the survey. This will help ensure a
high rate of participation and create an accurate national estimate of student behaviors.
