OMB No. 1625-0127

Exp: 04/30/2021

Who must comply?

Operators of certain commercial vessels and facilities.

What is this collection about?

This information collection voluntarily asks an operator to provide Essential Elements of Information (EEI) about their vessel or facility to the Coast Guard for use in Marine Transportation System (MTS) Recovery planning. Following a port disruption, Facility Status information is needed to determine the best course of action for port recovery.

Where do I find the requirements for this information?

Coast Guard guidance about MTS Recovery Planning and Operations is found in COMDTINST_16000.28B. A MTS Recovery Unit Fact Sheet is at this LINK.

For MTS EEI submissions, use form CG-11410.

For MTS Facility Status, use form CG-11410A.

CG public use forms are at this LINK.

When must information be submitted to the Coast Guard?

The MTS EEI is a one-time submission when a vessel or facility begins operation or changes their operation. The MTS Facility Status report is submitted in a port impacted by a transportation security incident or natural disaster.

How is the information submitted?

All the above information may be submitted in writing or electronically via facsimile or e-mail.

What happens when complete information is received?

This information will be reviewed by CG port safety/security specialists. No specific documentation will be issued by the CG.
