To determine continued entitlement to benefits as a full-time student, up-to-date information is
needed about
school attendance, marital status, and expected earnings. Therefore, it is
important that the steps outlined below be carefully followed when completing the enclosed Form
G-315, Student Questionnaire.
Step 1 - Complete Section A.
Step 2 - Take the form to the *ChooseOne* who must:
 review the
furnished in Section A,
 enter the required information in Section B,
 sign the form in Item 12, and
 promptly return the form to you.
Step 3 - If Form AA-19, Application for Child's Annuity, is not being filled out at this time, complete
Section C of the Form G-315.
Step 4 - Sign the Student Certification in Section D of the Form G-315.
Step 5 - Return the completed form within 30 days in the envelope provided. Do not return by
email or facsimile. Failure to return these forms by
, may result in suspension of
benefits. Remember, you are responsible for repaying any undue benefits
For months
age 18 through age 19, complete a separate Form G-315 for each school year.
If
schools during the school year, complete a separate Form G-315 for each school. If
needed, additional forms may be obtained from any Railroad Retirement Board office.
