Attestation for Commercial Transportation of Disembarking Crew
for Cruise Ship Operators under a No Sail Order Response Plan
This attestation is only applicable to cruise ship operators who have previously signed the
Acknowledgement of No Sail Order Response Plan Completeness and Accuracy and for
their ships that have met CDC’s criteria for a “Green” ship. Prior to receiving permission to
disembark crew by commercial transport, the cruise company signs this unaltered attestation
and provides it to CDC at [email protected]. Once CDC has received and cleared the
attestation, CDC will post the name of the ship on its website to inform transportation
partners (e.g., Federal Aviation Administration, Transportation Security Administration, and
airlines) that the cruise ship operator may arrange commercial travel for crew from that
ship. The attestation will be valid from the time of CDC’s clearance until the ship’s status
changes due to a breach in the conditions of the attestation, or the end of the NSO period.

The extension to the No Sail Order signed by the CDC Director on April 9, 2020, and effective on April
15, 2020, requires cruise ship operators to submit an appropriate, actionable, and robust plan that
adequately prevents, mitigates, and responds to the spread of COVID-19 on board cruise ships. Cruise
ship operators who have previously signed the Acknowledgement of No Sail Order Response Plan
Completeness and Accuracy may use commercial transportation to repatriate or transfer crew only from
ships that are unaffected by COVID-19, i.e., ships that have met CDC’s criteria for a “Green” ship.
The President and Chief Executive Officer of the operating cruise company, the Chief Ethics and/or
Compliance Officer of the operating cruise company and all parent companies, and the highest-ranking
Medical Officer of the operating cruise company and all parent companies must sign the following
attestation with respect to the cruise ship from which crew are being disembarked:

conditions are true and correct to the best of my knowledge and belief

A qualified medical professional has determined that there has
been no one on board with COVID-like illness1 or confirmed
COVID-19 within the past 28 days per the illness surveillance
requirements under No Sail Order response plan. Commercial
travel will not occur if COVID-like illness or confirmed
COVID-19 is detected.

•

If this ship received ship-to-ship2 transfers within the past 28 days,
crew must have come from a ship that had no confirmed
COVID-19 or COVID-like illness within the 28 days before the
transfer occurred.

COVID-like illness means acute respiratory illness (ARI), influenza-like illness (ILI), or diagnosis of pneumonia.
Ship-to-ship transfers are defined as any movement of crew between ships.

Per the No Sail Order response plan, medical staff will screen
disembarking crew members for signs and symptoms of COVIDlike illness prior to their disembarkation, using temperature
checks, visual observation for illness, and use of health
questionnaires.

•

Per the No Sail Order response plan, the cruise ship has followed
the most current CDC recommendations and guidance 3 for any
public health actions related to COVID-19, including but not
limited to those related to providing cloth face coverings to
disembarking crew members.

•

The cruise ship operator will ensure any land-based embarking
crew are quarantined for 14 days immediately upon embarking the
ship.

I acknowledge that any false or misleading statements or omissions may endanger health
and safety, including but not limited to the loss of lives and other irreparable harm.
Therefore, false or misleading statements or omissions may result in criminal and civil
actions for fines, penalties, damages, and imprisonment.

COVID-19 and are subject to change. The signer of this document is attesting that to the best of his/her knowledge and
belief it is true and correct that the cruise ship has complied with CDC’s recommendations and guidance for any public
health actions related to COVID-19 that are publicly available as of the date of signature.
