THE PAPERWORK REDUCTION ACT OF 1995

Public reporting burden for this collection of information is estimated to average 5 minutes per response, including the time for reviewing instructions, gathering and maintaining the data needed, and reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. The information collected through this grantees feedback survey will provide ACF with information about how it can continuously improve its data capacity building activities to meet the Sexual Risk Avoidance Education grantees’ needs. The OMB number for this information collection is 0970-0401 and the expiration date is XX/XX/XXXX.











The table identifies questions to gather feedback from SRAE grantees on a range of data capacity building (DCB) activities. Responses to the proposed feedback questions will be gathered primarily through three means: (1) the registration process for webinars to capture data before the event, (2) polls during webinars and (3) web-based surveys. Surveys may be administered to grantees 1) after webinars or one-on-one support, 2) after downloading written materials on the SRAENE website, or 3) before or after participation in pilots or reviewing videos on the SRAENE website1.
