TITLE OF INFORMATION COLLECTION: Main Beta v2 Survey
PURPOSE OF COLLECTION:
What are you hoping to learn / improve? How do you plan to use what you learn? Are there artifacts (user personas, journey maps, digital roadmaps, summary of customer insights to inform service improvements, performance dashboards) the data from this collection will feed?
The purpose of this collection is to measure visitor satisfaction with the IRS.gov desktop site. Information collected will be used to provide meaningful feedback to managers and staff to aid in prioritizing feature and usability enhancements. The surveys will assist in assessing overall customer satisfaction and overall quality of the site.

Who will you collect the information from?
Explain who will be interviewed and why the group is appropriate for the Federal program / service to connect with. Please provide a description of how you plan to identify your potential group of respondents and if only a sample will be solicited for feedback, how you will select them(e.g., anyone who provided an email address to a call center rep, a representative sample of Veterans who received outpatient services in May 2019, do you have a list of customers to reach out to (e.g., a CRM database that has the contact information, intercept interviews at a particular field office?)
Survey respondents will be visitors who access IRS.gov from a desktop device. These visitors are appropriate for this survey because the goal is to collect feedback about the experience of using the site from a desktop device. A sample of 50% of users who visit the site from a desktop device will see a popup inviting them to complete the survey. After receiving the invite, a minimum of 30 days must pass before a user will become eligible to receive another invite.
How will you ask a respondent to provide this information?
(e.g., after an application is submitted online, the final screen will present the opportunity to provide feedback by presenting a link to a feedback form / an actual feedback form)
An invitation popup will be presented to 50% of IRS.gov visitors. Users may opt-in to the survey or dismiss the popup.
What will the activity look like?
Describe the information collection activity – e.g. what happens when a person agrees to participate? Will facilitators or interviewers be used? What’s the format of the interview/focus group? If a survey, describe the overall survey layout/length/other details? If User Testing, what actions will you observe / how will you have respondents interact with a product you need feedback on?
If the user opts-in to participate, users will see a standard survey with 14 questions, including the questions required per the A-11 Section 280 guidance. Facilitators and interviewers will not be used.
This survey includes two free response questions. Free response question #3.1 will be used to better understand why a respondent’s need was or was not addressed with the transaction. Free response question #10 will be used to better understand what improvements could be made to IRS.gov. IRS is only using the data from the open-ended questions qualitatively to inform the responses in questions 3 & 10.
Please provide your question list.
Include as an attachment the questions or prompts presented to participants in your activity. Make sure that all instruments, instructions, and scripts are submitted with the request.
