INSTRUCTIONS
(1) Every proprietor of a distilled spirits plant must prepare this form monthly, in duplicate.
(2) Forward the original to the Director, National Revenue Center, 550 Main Street, Ste. 8002, Cincinnati, OH 45202-5215, not
later than the 15th day of the month following that for which the report is rendered. You should retain the remaining copy
for your records.
(3) Further instructions and additional guidance for completing this form are available on the TTB website at
