Cleaning and disinfection, depopulation and disposal, and appraisal are covered on the following
series of screens that show what type of work was performed, the materials used, and when the
work was completed:

The electronic forms are completed by a State or Federal diagnostician, not by the owner or
manager of the premises being investigated. The owner or manager verbally provides
information for input into the EMRS. These are diagnostic investigations, conducted as question
and answer sessions, designed simply to determine the nature and extent of illness on the
premises so VS can take the necessary steps to control and eradicate a foreign animal disease
outbreak.
EMRS accommodates multiple methods of entering data. APHIS staff or State personnel
accessing and using EMRS can build personal user and contact screens including their contact
information, dispatch information, and deployment readiness checklist.

Potential FAD incident sites can be tracked by general contact information:

Premises ID screen (includes location and investigation and quarantine status):

Livestock Structure Parameters screen:

Animal Business screen (includes location and contact information, types of animals handled,
and production type):

Animal Group screen (includes location, species, owner, and status):

Or Individual Animal screen (includes location, type of animal, ID, and full description,
including animal ID):

Animal ID screen:

The ID screen includes the ID number, the type of ID, the date applied, the type of animal and
the animal’s gender.
