Revision to VS Form 16-78

The USDA APHIS Veterinary Services (VS) revised its field operation procedures and simplified the information reporting requirements for service centers. As a result, Fields 13a and 14a of VS Form 16-78 (MAY 2020) were revised to clarify the information being collected.
Under the old form, respondents selected a service center from a series of check boxes and then typed in the address and contact information, before checking another series of boxes selecting notification method, and then typing in the date notified. They did this twice, once each for Fields 13a and 14a.
Under the new form (VS Form 16-78 (JUL 2021)), respondents now only type the state or territory of the material’s destination, and the email address of the responsible VS local office, as well as select the notification date from a dropdown calendar. They continue doing this twice, once each for Fields 13a and 14a.
The estimated response time for completing the form remains 10 minutes.
VS Form 16-78 is used in ICR 0579-0040, 0245, and 0307.
