Dear Driver,

The Centers for Disease Control’s (CDC) National Institute for Occupational Safety and Health (NIOSH) will be here at ______________________ (Location) on ____________________(Date) to conduct a study to learn more about how work in the local/short-haul (L/SH) industry relates to truck driver health & safety. We would like to invite you to participate in this study by completing a safety and health survey related to L/SH truck driving. All L/SH drivers at this location will be asked to participate. In order to participate, we ask that participants be employed full time as a L/SH truck driver for at least six months.

The mission of NIOSH is to improve worker safety and health through research and education. NIOSH does NOT enforce any laws. Kennesaw State University and the University of Georgia will be facilitating the data collection process. We plan to disseminate the summary results to the L/SH trucking industry at large and the scientific community to help improve L/SH truck driver safety and health.

Your participation is voluntary and anonymous. If you choose to participate, the survey can be administered either in hard copy or electronically, and should take you about 30-40 minutes to complete. You may choose to answer any or all questions. You may drop out any time for any reason without consequence to you. We will provide you with a token of appreciation for completing the survey.

There is minimal risk to participate in this survey. Your answers will be anonymous. You benefit by having the opportunity to share your thoughts about your work which may increase knowledge about the health and safety issues facing all local/short-haul truck drivers, and help improve working conditions for drivers in the future.
