Introduction to the Unmanned Aircraft System (UAS) Maintenance Report
The UAS Maintenance Actions Monthly Report is the means for the UAS Integration Office and
other Federal Aviation Administration (FAA) offices with the responsibility of safely integrating
UAS into the National Airspace System (NAS) to gather summary UAS corrective maintenance
data. The data is correlated to operational flight data and accident/incident/occurrence/anomaly
data to identify trends related to UAS durability and reliability. The data will assist with setting
performance standards and informing future rulemaking.
The form also complies with Part 107 waivers that include a requirement to submit a monthly
report of equipment degradations, malfunctions, or failures to the Administrator at [email protected]. Upon submission of this form, the UAS Integration Office generates
the output report required by the Flight Standards Office.
Under 14 CFR Part 135.415, Part 135 operators must submit Service Difficulty Reports (FAA
Form 8070-1). Participants in the Partnership for Safety Program (PSP) and BEYOND program
operating under Part 135 submit Service Difficulty Reports in lieu of this UAS Maintenance
Report.
Populate the UAS Maintenance Report Form
Identifying Information
1. Participant Name. Provide the name of the Participant (PSP) or Lead Participant
(BEYOND).
2. Company Name/Operator (optional). The participant may choose to document the
company name or operator for their own recordkeeping purposes. This information will
not be captured or used by the FAA.
3. Month. Select the month in which the maintenance occurred.
4. Year. Select the year in which the maintenance occurred.
5. Time Zone Used to Record Times. Indicate whether the time in the table below is local
time or coordinated universal time (UTC) time.
Maintenance Summary
1. Date. Enter the date of the equipment degradation, malfunction, or failure.
2. Time. Enter the time of the equipment degradation, malfunction, or failure. If using a 12hour clock, include am or pm to designate a morning or afternoon flight.
3. Aircraft Registration Number or Nickname. Provide the unique identifier of the aircraft that
experienced some type of equipment degradation, malfunction, or failure. Depending on
the weight of the aircraft and when the aircraft was registered, the unique identifier may
be the nickname assigned to the aircraft when registering it in DroneZone or it may be
the registration number assigned to the aircraft when registering it using AC Form 80501.
4. Description of Equipment Degradation / Malfunction / Failure. Provide a brief description
of the component that failed, how it failed, and what caused it to fail (if known).
5. Subsystem Type. From the drop-down box, select the type of subsystem (avionics,
communication, etc.).
6. Time in Service (hours). Enter the number of hours that the aircraft has been in service
(total flight time).
7. Maintenance Action. Provide a brief description of the repair and/or replace actions
required to return the aircraft to service. Identify the components repaired or replaced.
