Dear Employer:


Several weeks ago, we invited you to complete the 2022 Survey of Respirator Use and Practices. The previous survey invitation was sent by postal mail and email. Unfortunately, we have not yet received a response for your establishment. We are sending this reminder (and one by email) because of the importance of your response.


Complete your survey by {DATE} following the instructions enclosed with this letter.


What is the purpose of this survey? The U.S. Bureau of Labor Statistics (BLS) and the National Institute for Occupational Safety and Health (NIOSH) are working together to determine how the COVID-19 pandemic affected the use of respirators and other respiratory devices, determine how employees are using respirators in the workplace, identify effective use of respirators, and develop effective educational materials.


Who should complete this request? Anyone knowledgeable about the use of protective respiratory devices by employees can complete the survey. If you are not knowledgeable about the use of protective respiratory devices in your establishment, then deliver this to a person at your establishment who is.


Is this part of an enforcement effort? Participation by your establishment is voluntary and results will not be used in any type of enforcement activity. This survey is conducted for informational purposes only. All responses will remain confidential and used only for statistical purposes.
