The information you provide is voluntary. Your home can be enrolled in the program whether you decide to participate or not. The public reporting burden for collection of this information is estimated to be 25 minutes per response. HUD may not collect this information, and you are not required to complete this form, unless it displays a currently valid OMB control number.

Instructions: This home hazard checklist is designed to be completed by the OT but can be completed by other grantee staff. In general, answer questions based on your observations but ask the client if clarification is needed.

Suggested Script: “Thank you for meeting with me today. This walkthrough will take about 25 minutes. Do you have any questions before we begin?”
