Dear (Health Department Point of Contact):


The Centers for Disease Control and Prevention (CDC), Division for Heart Disease and Stroke Prevention (DHDSP) is seeking your participation to gather in-depth information as part of the case study about the Category B core program area, [Clinical Quality Measures, Team-Based Care, and Community Clinical Linkages]. This case study is a component of the comprehensive evaluation for the DP-18-1815 cooperative agreement. The case study will provide insight on the implementation of the [insert topic area], including facilitators and barriers to implementing the strategies and examples of common and unique activities implemented to address the strategies.


As part of this assessment, we would like to conduct state health department (virtual) site visits comprised of video and/or phone interviews and group discussions with members of your [insert topic area] team. The interview will take approximately 1.5 hours to complete and the group discussions will take no more than 2 hours. The interview and group discussion guides contain short-answer questions about your state health department’s activities related to [insert topic area]. To assist us in scheduling the interviews, we ask that you please respond to this e-mail by [date] with 3 times and dates that fit your schedule.


Your participation in this interview is completely voluntary. You may choose to skip questions or stop the interview at any time—that will not in any way impact the funding or technical assistance you receive from CDC. The interview will be recorded for transcription purposes. All information collected will be kept secure and reported out in aggregate form.


The findings from the case study may be used to inform future programming, funding opportunities, guidance documents, and identify promising practices that can be replicated and scaled to better improve health outcomes.
