In the quality section of the
online reporting tool, the
findings of monitoring
section requires the state to
indicate if deficiencies were
detected. If so, a summary
of the significant areas where
deficiencies that were
detected is provided by the
state in the text box.

The findings of monitoring
section will list each quality
performance measure approved
in the waiver for the state to
enter the specific performance
data.

Revision

Reporting on all of the
waiver's quality performance
measures on an annual rolling
basis will allow early
detection of deficiencies so
more timely action can be
taken. It will also eliminate
the need to provide three
years’ worth of data during
the waiver cycle. Rather the
data provided in the annual
reports will be utilized for the
full quality review.

Yes. Overall burden
will be decreased. States
were already required to
gather and analyze the
performance measure
data annually, but were
only required to report
deficiencies on the 372
report. There is a slight
increase in burden due
to additional data
required. The increase is
offset by eliminating the
need to provide three
years of data at once.

The findings of monitoring

deficiencies will list each
performance measure that does
not meet 86% compliance or
an otherwise agreed upon
compliance threshold
approved by CMS.

deficiency, information about
how the deficiency is being
