special instructions on this assignment. If you do not receive a response within 24 hours, conduct an on-site
investigation as follows:
Flame jetting is a flash fire incident involving a flammable liquid fuel within a container that is poured near a flame
or other flame source. Products that can be involved in a flame jetting incident include, but are not limited to,
fireplace fuels, gasoline containers, firepot fuels, fire pit fuels, fire starters, lighter fluids, solvents, and alcohol
containers.
Please obtain any fire incident reports and any other reports regarding the cause of this incident.
Provide a complete product identification of the fuel used in the incident including the age, manufacturer, brand,
date manufactured, place of purchase, model or serial number, and chemical composition and/or fuel type.
Provide a complete product identification of the fuel container (if not sold pre-filled, such as a gasoline container)
used in the incident including age, manufacturer, brand, date manufactured, place of purchase, model or serial
number, and whether or not a flame arrestor or screen was present in the mouth of the product.
Provide a complete product identification of any other products involved in the incident such as a fire place, fit pit,
or electric device including age, manufacturer, brand, date manufactured, place of purchase, and model or serial
number.
If possible please photograph all products involved in this incident. If possible please collect sample of the incident
fuel container.
Document the incident details including:
• Did the incident happen outside or inside?
• What were the illumination characteristics (e.g. outside in the sun or at night or inside in bright light)?
• What was the consumer doing with the fuel prior to the incident? Was the consumer pouring fuel?
• What was the fuel being used for? Was there a flame, hot surface or other ignition source present? If not,
could a hidden ignition source been present?
• What was happening immediately prior to the incident?
• Was there any delay between using the fuel and the incident? If so, what were the sequence of events and
approximate timing?
• Approximately how full was the fuel container prior to the incident?
• Was the fuel container damaged in the incident? If so, what was the damage?
• Was anything else damaged in the incident? Is so, what was the damage?

Document the victim details including:
• How many people were injured?
• What were the injuries? What body parts were injured? Degree of the injuries? If burned, to what degree
and what percentage of total body surface area?
• Was the victim(s) using the fuel at the time of the incident?
• How far was the victim(s) from the source of the flame jetting incident?
• Was the victim(s) clothing ignited? If so, what was the condition of the clothing?
• Was the victim(s) admitted to the hospital? If so, for how long?
• Has the victim(s) needed or is likely to need skin grafts?

Please include all primary and all backup contacts in the distribution of the completed IDI.
