ASSIGNMENT MESSAGE:
1. If the incident involved a candle trim ring, candle burner, potpourri burner, or some other candle accessory
besides a typical holder or container, contact the victim to determine whether any part of the incident candle and/or
accessory is still available. If so, conduct an on-site investigation and try to collect the candle and accessory, or any
remaining parts of the candle and/or accessory. If not, try to collect an identical candle and/or accessory.
If the incident did not involve a candle ring or some other unusual candle accessory, conduct a telephone interview.
2. Please investigate the attached incident in accordance with the guideline “Candle Fires and Fire Hazards.” Focus
attention on (1) any unusual behavior exhibited by the candle, (2) victim interaction with candle, and (3) use
characteristics, especially as regards to candle accessories.
Verify incident scenario. Provide complete product identification including: manufacturer, brand, number of
wicks, date manufactured, place of purchase, date of purchase, model/serial numbers, cost, etc. (There is an
increase in candles with multiple wicks and incidents with multiple wick candles. ASTM is trying to figure out if
multiple wicks effect the possibility of an incident.)
Please use Guideline 121 for Candles as well as Guideline 129 for Smoke Alarms, Sprinklers, and Fire
Extinguishers.

Please include all primary and all backup contacts in the distribution of the completed IDI.
