ASSIGNMENT MESSAGE:
Only serious injuries and deaths should be investigated. For any entrapment incidents involving
an automatically controlled garage door which failed to reverse to the open position when
coming into contact with an object (person, animal, etc.) in its downward path, conduct an indepth investigation. Confirm the incident scenario, the victim's age, sex, activity at time of the
incident, etc. Find out when the automatic door opener was purchased, where it was purchased,
and by whom (the homeowner, installer if not the homeowner, etc.); when it was installed and
who installed it; when the automatic door opener was manufactured and by whom.
Determine if the opener claimed to meet the voluntary UL entrapment protection standard
(models manufactured 1982 - 1990); the 1991 CPSC standard, or the 1993 CPSC standard.
Determine when the automatic opener system was last adjusted, and/or tested for proper
operating of the entrapment protection system. Collect any available official records associated
with the incident. Describe and/or photograph any product labeling. Was the emergency release
used during the incident?
