ASSIGNMENT MESSAGE:

For internet of things (IoT)/connected product incidents where the internet or wireless network
connection has led to the incident or risk of safety hazard, describe the safety hazard. Explain
how the remote connection to the internet or other wireless network or software vulnerablility
caused the safety hazard.
Using Appendix 140, as well as any other applicable Appendix/Guideline, please conduct an
investigation for this IoT/connected product incident. Describe the scenario surrounding the
incident. Obtain pre-incident, incident, and post-incident information. Identify all products by
manufacturer and model numbers. Collect information on the smartphone or other controlling
devices (tablet, PC, or other) and software versions used on the internet connected device (e.g.,
smart toys, internet/app connected appliances, smart wearables, micro-e-mobility devices, home
security/safety systems, etc.) and controlling devices. If possible, collect owners/user’s manual
for all appliances involved.
OBTAIN COPY OF FIRE REPORT, MEDICAL EXAMINER’S OR ANY OTHER
OFFICIAL REPORT.
Please include all primary and all backup contacts in the distribution of the completed IDI.
