ASSIGNMENT MESSAGE:

Conduct an on-site investigation of the attached incident related to a HOME POWER TOOL.
Determine the following:
1) the accident scenario;
2) how the product was being used;
3) how the product and involved material had been positioned just as they were prior to the injury
(without power to the product), document with photograph;
4) the type and model of this particular product and type of wood or material being cut or used;
5) if the kickback of the wood contributed to the incident and if so, how;
6) if blade guard was in use and if use of blade guard would have prevented incident;
7) if blade guard was not in use or specifically removed, ask why;
8) take photos of the blade guard and any other safety devices present;
9) document how, where, and in what direction the victim’s hand or body part contacted the blade;
and
10) document if safety devices, such as contact triggers on nail guns, malfunctioned or were bypassed
by the consumer.
Be particularly watchful for altered or modified equipment. If changes have been made, find reason(s) for
changes. Photograph product I.D. and presence or absence of guards, protective devices, etc. Attach the
owner manual and medical and/or official reports that may be available on this incident. Use the attached
investigation guideline and/or data record sheet.
Please include all primary and all backup contacts in the distribution of the completed IDI.
