ASSIGNMENT MESSAGE:

Conduct an on-site investigation of the chain saw related incident discussed in the attached
document. Determine brand, model, and if possible, cubic inch displacement of the engine.
Also attempt to determine if the chain is a low kick back chain and if the saw is equipped with
any safety devices. Determine how and what the victim was cutting, document with photograph.
If available, obtain a copy of the owner's manual. In case of fatality, obtain the medical and/or
official reports.
Please include all primary and all backup contacts in the distribution of the completed IDI.
