Conduct an on-site investigation of the attached Circular saw related incident.
Determine the following:
a. the incident scenario
b. the position and movement of the product with respect to the material being cut just prior
to the injury (without power to the product) – document with photographs.
c. the material being cut and the type and model of the product involved
d. did kickback of the wood contribute to the incident?
e. did the blade guard operate properly, did any material jam the guard?
Photograph the blade guard.
Describe and/or photograph any product labeling.
Collect medical and official records associated with this incident in case of fatality.
Please include all primary and all backup contacts in the distribution of the completed IDI.
