For fatalities involving toys, include Yongling Tu, EPHA, in the distribution of the completed IDI.

Information on deaths and serious injuries associated with toys is always of interest to the
Commission. Findings from investigations continue to be used for various projects, as well as
public awareness campaigns during the holiday season. All deaths and serious injuries to children
associated with these products should be investigated.
Determine product description, brand name, model; photograph product if possible. Determine age
of product, manufacturer, and place of purchase. If choking hazards were involved, please obtain
the dimensions and materials of the object involved.
Determine sequence/scenario of events, treatment administered and patient disposition. If more
than one person was involved, explain how others were involved. Collect all official documentation
(paramedics, police, and coroner reports, medical record, and death certificates).
Determine if product failure or faulty product design was a factor.
Please include all primary and all backup contacts in the distribution of the completed IDI.
