ASSIGNMENT MESSAGE: Conduct an in-depth investigation of the UTV fatality reported in the attached
document using the UTV Investigation Guideline of Appendix 91. If you have any special concerns about this
investigation, please do not hesitate to call your headquarters contact.
A narrative write-up for this IDI is not required (including both ATV and UTV incidents), provided the DRS is
completed. If both an ATV and a UTV are involved in the incident being investigated, then both the ATVD and
UTVD DRS should be completed. An on-site investigation is not required for either ATVs or UTV incidents.
A Utility Vehicle (UTV) or Recreational Off-highway Vehicle (ROV) is a vehicle that has a steering wheel,
seatbelt or other restraint, , roll bar, cargo bed, foot pedals, or bench or bucket seats, excluding dune buggies
and golf carts. UTVs are also known as “side-by-side ATVs” indicating that the seating is side-by-side
instead of straddle.
Below is a list of vehicle make and models where the vehicle is not an ATV and the UTV investigation guidelines should be used. Please note
this is not a comprehensive list of all makes and models of UTVs, but a list to help facilitate the identification of common UTVs.
