ASSIGNMENT MESSAGE:

Conduct an IDI on the attached report involving a sports-related death or serious injury.
Specify the sport and the level of play (organized game, backyard play, tournament, etc.).
Specify presence or absence of any relevant protective equipment. Try to get information about
the age and size of any other players involved (age of pitcher if thrown baseball, age of tackler in
football, etc.). Try to determine the speed of impact if applicable, how fast was the ball going,
how fast was the person running, etc.).
Collect any medical, coroner's and any other official reports available. Please get the autopsy
report for deaths, if at all possible, since this is particularly valuable in determining the true cause
of deaths.
If death or injury resulted from an equipment failure, try to collect the equipment.
Please include all primary and all backup contacts in the distribution of the completed IDI.
