T ITLE OF INFORMATION COLLECTION: User satisfaction data collection for the Indian Health Service Office of Information Technology National Data Warehouse.


PURPOSE: The purpose of this data collection is to determine user satisfaction of the Indian Health Service (IHS) Office of Information Technology (OIT) National Data Warehouse (NDW). HHS mandates that each IT investment perform an Operational Analysis on an annual basis. One important aspect of an Operational Analysis is to gather information from users, to determine if the enterprise tools that are currently in place still meet their needs.


The information gathered in this data collection will be reported to OIT Management, including the IHS Chief Information Officer (CIO), to communicate the need for possible process or technological improvements to the NDW.



DESCRIPTION OF RESPONDENTS:

The targeted group for this data collection is users of the National Data Warehouse. The survey will be sent to a specific list of users to solicit feedback on their experience with the NDW and to provide comments and recommendations at their discretion. Participation is entirely voluntary. The collection of PII data is optional and will only be used in the event that a respondent requests an answer to an inquiry.

FEDERAL COST: The estimated annual cost to the Federal government is $140.90. One IHS employee at a GS-13/7 with a salary of $70.45 per hour (includes 36.25% fringe benefit increase) reviews and collates data from the surveys-taking two hours per year.

TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request (e.g., Comment card for soliciting feedback on xxxx).


PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include a statement to that effect in your explanation. Please include how the information will be used to improve services or the program.


DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.


TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.


CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the collection will be returned as improperly submitted or it will be disapproved. Provide the name of the individual who is the lead contact and responsible for the collection.
