T ITLE OF INFORMATION COLLECTION: OMB Form No. 0917-0036: Chinle Service Unit (CSU) Adult Care Unit (ACU) Patient Experience Survey


PURPOSE:

To support the IHS mission, the information obtained from the inpatient satisfaction surveys provides feedback to the provider and care teams about patient’s hospital experience of inpatient care. The information gathered will be used to measure patient satisfaction, identify strengths and weaknesses in current service provisions, improve patient care, and make other improvements that are practical and feasible for CSU service programs.


DESCRIPTION OF RESPONDENTS:

The ACU Inpatient Satisfaction Survey is administered by the staff to a patients, or his or her designee to complete on behalf of the patient, on a voluntary basis upon discharge from the OCU unit. The survey contains a total of 20 questions based on the patient’s experience at the inpatient services of the facility. Staff will collect the surveys and enter the results into the Survey Monkey website on the date of visit for further review and analysis.

FEDERAL COST: The estimated annual cost to the Federal government is $1500.


If 1200 surveys collected for the overall year and it takes staff 3 minutes to process each survey, then it will take approximately 60 hours each year to gather data and report results. A GS 9 rate of about $25 per hour x 60 hrs. = $1500 per year.

Instructions for Completing Request for Approval under the “Generic Clearance for the Collection of Qualitative Feedback on Agency Service Delivery”


TITLE OF INFORMATION COLLECTION: Provide the name of the collection that is the subject of the request (e.g., Comment card for soliciting feedback on xxxx).


PURPOSE: Provide a brief description of the purpose of this collection and how it will be used. If this is part of a larger study or effort, please include a statement to that effect in your explanation. Please include how the information will be used to improve services or the program.


DESCRIPTION OF RESPONDENTS: Provide a brief description of the targeted group or groups for this collection of information. These groups must have experience with the program.


TYPE OF COLLECTION: Check one box. If you are requesting approval of other instruments under the generic, you must complete a form for each instrument.


CERTIFICATION: Please read the certification carefully. If you incorrectly certify, the collection will be returned as improperly submitted or it will be disapproved. Provide the name of the individual who is the lead contact and responsible for the collection.
