via the SAMS messaging system.
This form is to be used for these non-formulary drugs: Hysingla ER (hydrocodone), Embeda (morphine sulfate;
naltrexone), Arymo ER (morphine sulfate), MorphaBond XR (morphine sulfate), Xtampza ER (oxycodone), RoxyBond
(oxycodone).
