Description
This generic information collection will collect sensitive or personally identifiable information (PII)
(name, address, phone number, email address, gender, race, ethnicity, and age group) during the
three-minute recruitment process (Attachment C1). A person's gender, race, and ethnicity helps to
determines if a respondent is eligible for the one- to two-hour discussion (either via focus group or
in-depth interview). A person's name, address, phone number, and email address is used to help
schedule the focus group or in-depth interview.
The recruitment and longer discussions will be managed by a communications contractor on behalf of
CDC. Neither CDC or the contractor will develop an an IT system or online data collection system for
this work. The PII will not be linked to the data collected during the one- to two-hour discussions. A
minimum amount of demographic data may be retained to help report novel findings, but not be
sufficient to identify respondents. Participants will be informed that the discussions will be recoded (by
video and/or audio) and transcribed. The DVDs with the discussions will be kept in a locked cabinet.
All DVDs will be destroyed 12 months from the date of the discussion. Only staff involved in this
project will have access to the recording and transcribed documents.
The consent form is included in this package as Attachment C2.
