Explanation of Non-Substantive Changes to OMB Control Number: 3060-0819:


  • FCC Form 5629

  • FCC Form 5630



Purpose of this Submission: This submission is being made for non-substantive changes to an existing information collection pursuant to 44 U.S.C. § 3507. This submission seeks to modify FCC Forms 5629 and 5630 to reflect updated information on the Federal Poverty Guidelines and an updated reference to the Lifeline System of Records Notice on the FCC’s online Privacy Act Information page. In addition, Forms 5629 and 5630 are being updated to make minor wording corrections and changes, as described below. The FCC’s Wireline Competition Bureau, pursuant to consultation with the Office of Management and Budget, will provide a 14-day opportunity for interested parties to comment on the proposed changes to FCC Forms 5629 and 5630.


Summary of Proposed Changes


FCC Form 5629 is being updated to include the following changes:

  • On page 1, adding the word “the” under “You May Need to Show Other Documents” (numbered paragraph 1).

  • On page 1, in the “Apply” box, adding the address where consumers can mail their application forms. New text states:

Mail the form to this address:

Lifeline Support Center

P.O. Box 7081

London, KY 40742”


  • On page 3, where it states, “Check if you live on Tribal Lands,” changing the uppercase “L” to a lowercase “l” in “lands.”

  • On page 3, changing the reference to the “Commission” to “FCC”.

  • On page 5, in Section 3, changing “All 48 States & DC” to “All 48 States, DC, and Territories”.

  • On page 5, updating Section 3 with the 2022 Federal Poverty Guidelines.

  • On page 6, correcting Section 4, after the third initial box (number 2) to change “including,” to “including” (removing the comma).

  • On page 8, changing the reference to the “Commission’s” to “FCC’s”.

  • On page 8, updating the “Purpose” in the Notice section with a more current reference to the Lifeline System of Records Notice, which will now reference the FCC’s online Privacy Act Information page.
