AGENCY DISCLOSURE NOTICE
The public reporting burden for this collection of information, 0702-0060, is estimated to average 266 minutes (25 minutes for Pre-Candidate phase, 195 minutes for the Candidate
Phase and 46 minutes for the Accepted Candidate phase) per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining
the data needed, and completing and reviewing the collection of information. Send comments regarding the burden estimate or burden reduction suggestions to the Department of
Defense, Washington Headquarters Services, at [email protected]. Respondents should be aware that notwithstanding any other
provision of law, no person shall be subject to any penalty for failing to comply with a collection of information if it does not display a currently valid OMB control number.

44, United States Code, Public Printing and Documents, Ch 31, Sec 3101; Executive Order 9397, Numbering System for Federal Accounts Relating to Individual Persons.
PRINCIPAL PURPOSE: Collection of data on Academy candidates for opening a file. ROUTINE USE: To gather information on a candidate in order to open a file for admissions to
the United States Military. DISCLOSURE IS VOLUNTARY. However, failure to provide information could preclude appointment. In addition to those disclosures generally permitted
under 5 U.S.C. 552a(b) of the Privacy Act of 1974, these records contained therein may specifically be disclosed outside the DoD as a routine use pursuant to 5 U.S.C. 552a(b)(3)
as follows: Academic transcripts may be provided to educational institutions for the purpose of admissions to further educational degree programs. The DoD Blanket Routine Uses
set forth at the beginning of the Army's compilation of systems of records notices also apply to this system.

Thank you for your interest in The United States Military Academy (USMA). Your name has been sent to a Field Force
Representative

in your area.  Field Force Representatives are the USMA Admissions Office’s representatives in local communities
nationwide and

overseas. They are an excellent resource for candidates because many of them are active duty or retired U.S. Army
officers who have

been trained in the USMA Admissions process. They are highly motivated to assist you and have a wealth of knowledge
to help you navigate

the Admissions process and many are Graduates themselves. They can also keep you informed about future West Point
events in your area 

and answer questions you might have.

You also have an Admission Officer at West Point who has been designated to assist you.  For administrative purposes,
the USMA Admission

office divides the country into specific areas of responsibility for these Admissions Officers, this is why they are known as
a Regional

Commander.  Each Regional Commander leads a team comprised of Admissions Technicians and Clerks who also have
the 

responsibility to assist you with the specific needs you may have during the application process. While the Field Force
Representatives 

should provide you answers to general questions, the Regional Commander and his/her team will be the ones to answer
questions about your 

specific application.  Whether it is related to the Candidate Fitness Assessment (CFA), a Nomination, your Department of
Defense Medical 

Examination Review Board (DoDMERB) physical, or any other specific issue, your Regional Commander stands ready to
assist.

Since each Regional Commander is responsible for many candidates we naturally encourage candidates to attempt to
answer relatively 

routine questions for themselves by contacting the Field Force Representatives or by using our web site, social media, or

a question that you
