MEMORANDUM FOR DEFENSE HEALTH AGENCY, CLAIMS COLLECTION
SUBJECT: Justification for the Use of Social Security Numbers on DD Form 2527, Statement
of Personal Injury - Possible Third Party Liability - Accepted
Thank you for completing a review of Social Security Number (SSN) use within this
form. The Defense Privacy, Civil Liberties and Transparency Division (DPCLTD) has reviewed
and accepted your request to use the SSN for the following Acceptable Uses: (8) Computer
Matching and (13) Other Cases. The SSN is collected to interface with non-Department of
Defense (DoD) federal agencies (Department of Veterans Affairs, Department of Health and
Human Services, etc.) for the purpose of filing third party liability (TPL) claims and to file TPL
claims with non-DoD entities (i.e. third party insurers) on behalf of DoD beneficiaries. The nonDoD entities will use the SSN to identify the subject DoD beneficiary. This form must be
reviewed on a continuous basis and a new justification must be provided if any change to the
form is submitted.
Acceptable Use (6) Administration of Federal Workers’ Compensation, is not authorized.
Significant justification was not provided for SSN collection in this case.
While this form has been identified as requiring the collection, maintenance and use of the
SSN, you must ensure this data is afforded the highest protections practicable through use of
appropriate administrative, technical and physical safeguards. Also, please maintain a copy of
your justification and this memo in your records.
SSN Justifications must be renewed three (3) years after the date of this memorandum if
the associated form has not been updated.
