Justification for Change
This is a request for nonmaterial/non-substantive change to Form 4A_SectionsAB and Form 4A_SectionsCD. The changes are as follow:
• Form 4A_SectionsAB – added “Zip code for case/patient/sample” to B7 to match the statement on CD.
• Form 4A_SectionsCD – added the word “reported” to D2 to clarify the statement
These changes are proposed to enhance clarity for the respondents. The proposed changes will not change the burden of this ICR.
