The National Marine Fisheries Service requires this information for the conservation and management of marine
fishery resources in accordance with the Magnuson-Stevens Fishery Conservation and Management Act. The data
reported will be used to develop, implement, and monitor fishery management strategies and for a variety of other
uses. Submission is mandatory for those persons falling under the requirements of 50 CFR 648. 7 (b ). All data
submitted will be handled as Confidential material in accordance with NOAA Administrative Orders. Public reporting
burden for the survey is estimated to average 12.5 minutes per response, including the time for reviewing
instructions, searching existing data sources, gathering and maintaining the data needed, and completing and
reviewing the collection of information. Send comments regarding this burden estimate or any other aspect of
this collection of information, including suggestions for reducing this burden, to NMFS, One Blackburn Drive,
Gloucester. MA 01930.
Notwithstanding any other provision of the law, no person is required to respond to, nor shall any person be subject
to a penalty for failure to comply with, a collection of information subject to the requirements of the
Paperwork Reduction Act, unless that collection of information displays a currently valid 0MB Control Number.

INSTRUCTIONS


IMPORTANT - This form must be maintained during this fishing season or until the fishing permit
is returned to the Regional Administrator of the National Marine Fisheries Service, whichever
occurs first. This report form is designed so that at the end of the weekly reporting period,
you simply remove the original card, marked "NMFS COPY", fold, seal, and drop it in the
mail. No postage is required. The duplicate copy, marked "YOUR COPY", will remain in the
book and should not be removed. You are required to report the previous seven days'
activities by mailing the form(s) at the end of every week during the fishing year, even if you
do not fish during the week.

Fill in the blank spaces at the top of each sheet with the required information before mailing.

2.	

Complete one row for each fishing trip, entering the date of the trip in the first column.

3.	

In completing the "Area Fished" column, you may enter either latitude and longitude
coordinates, the commonly used geographical name of the area, or the appropriate LORAN
C bearings.

4.	

The "Time at Sea" column should reflect the interval of time from leaving port to returning
to port.

5.	

The "Time in Fishing" column should include the total time in which the dredge is pulled
across the bottom.

6.	

The "Catch in Bushels" column is self-explanatory.

7.	

The "Average Discard Percent" column should specify the average percent of discards of
surf clams or ocean quahogs per tow for the trip.

8.	

The "Port Landed" column is self-explanatory.

9.	

The "Name of Buyer and Date of Sale" column is self-explanatory. Enter date as Month#/Day
(for example, June 14th would be 06/14).

10.	

The "Price per Bushel" column is self-explanatory.

11.	

The "Allocation Number" column should reflect the number of the allocation holder for whom
the vessel is fishing. If the vessel is being used to fish for more than one allocation in a trip,
use a separate line for each allocation number.

12.	

The "Tag Number" column may be entered as either individual tag numbers, or as the
beginning and ending numbers of a consecutive tag series used.

13.	

A space is provided at the bottom of each report form for you to write in any comments you
wish to make.

Any failure to accurate'ly and completely provide the information required herein constitutes
a violation of 50 CFR Section 652.8(b)(13) which could result in a civil penalty of up to $25,000
per offense, a permit sanction, and/or a loss of allocation.
