Hello, this is _______with _______Health Department. We are working on a project with the U.S. Centers for Disease Control and Prevention looking at how restaurants handle staff issues, such as sick employees. Your restaurant was picked at random to be a part of this project. I’d like to come to your restaurant and interview you about what you do at work and how the restaurant manages sick food workers.

I’d also like to take a brief look around the kitchen. What you say will be kept protected and once we have completed the study the link between your restaurant and the answers you provide will be deleted – at that time, it will not be possible to link anything you say with you or your restaurant. Your participation would be voluntary, but we would really appreciate your participation. It will help us understand the challenges sick workers pose for restaurants.  Whether you are part of the study or not will not affect your restaurant’s score on any health inspection.

Having said that, I need to let you know that at any time during the visit if I see something that is an imminent health hazard, such as no power, no water or sewage on the floor, I will need to stop the interviews and report the problem to your local health department.

This study will consist of two to three site visits over three to six months, each visit should take about an hour. On these visits, we will interview a manager and conduct a brief observation of workers handling food in your kitchen. We may also provide information about sick worker management plans to the manager.

Could I schedule a time to come out and speak with a manager? We’d like to speak with the manager with authority over kitchen workers. This could be a kitchen manager, general manager or owner.
