This is a gentle reminder that the deadline for completing the web-based Preventative Health and Health Services Block Grant Assessment is MM/DD/2022.
The purpose of this assessment is to collect data and information on results or improvements achieved through the PHHS Block Grant from July 1, 2021 through June 30, 2022. The findings from this assessment will be used to inform CDC of the outputs and cross-cutting outcomes of the PHHS Block Grant, refine existing measures, and inform the development of future measures. Responses will be kept secure and results will be reported only in aggregate form. The findings will constitute an important component of the overall assessment of the PHHS Block Grant.
