This is a final reminder that the deadline for completing the web-based Preventative Health and Health Services Block Grant Assessment is MM/DD/2022. Your feedback will inform CDC on the outputs and cross-cutting outcomes of the PHHS Block Grant, refinements to the existing measures, and the development of future measures. Responses will be kept secure and results will be reported only in aggregate form.
The questionnaire, available, via the link below, is voluntary and will take approximately 45 minutes to complete.
