FSTD Sponsor Notification Form FSTD Sponsors should use this form to notify the NSP of their intent to use a modified FSTD in accordance with §60.23(c) or to report the discovery/intent to incorporate new validation data in accordance with §60.13(f). Reference NSP Guidance Bulletin 11-02. Sponsors should complete sections 1 & 2. Limit one change per form. One change may affect multiple FSTDs.
