About a month ago, we sent you an invitation and a few reminders to take part in the U.S. Department of Agriculture’s (USDA) Family Child Care Home Provider Experience Survey.



You have not responded to the survey yet and we want to hear from you! We urge you to complete the enclosed paper survey and share your experiences with the Food Program, or the Child and Adult Care Food Program (CACFP), and recommendations on how this program could be improved for family child care providers like you.

Please complete the survey, even if you are no longer participating in the Food Program or no longer operating a family child care home.

Once you complete the survey, we will send you a $40 gift card.

The survey will take 20 minutes to complete. Your answers will be automatically saved so that you can stop and come back at any time.

This is a voluntary collection and FNS will use the information to provide technical assistance and inform program improvements to support family child care home participation in CACFP. This collection does not request any personally identifiable information under the Privacy Act of 1974. According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a valid OMB control number.
