A Message from the U.S. Census Bureau:
We are requesting your cooperation with the Manufactured Housing Survey. This survey is
conducted by the U.S. Census Bureau and sponsored by the Department of Housing and Urban
Development (HUD). It produces monthly regional estimates of the average sales price for
new manufactured homes, detailed annual estimates including selected characteristics of new
manufactured homes, and monthly estimates of homes shipped by status.
The enclosed form identifies the selected manufactured home in Section I. Please complete
Sections II through IV. Use the enclosed form as a worksheet prior to reporting online, or
return the completed form in the enclosed envelope. Online reporting is encouraged due to
cost savings and reduced processing times. Website and log in information can be found on
the front page of the form. You may also submit your survey by fax (1-800-274-0761). If we
do not receive your response by the due date printed on the form, we will contact you by
telephone.
YOUR RESPONSE WILL BE KEPT STRICTLY CONFIDENTIAL. We estimate this
survey will take an average of 20 minutes to complete. Additional information about the
authority, confidentiality, and burden of this data collection can be found on the back of this
letter.

Authority and Confidentiality
Title 42, United States Code (U.S.C.), 5424 note; Title 13, U.S.C., Section 8(b); and Title 12
U.S.C., Section 1701z-1, authorizes the U.S. Census Bureau to conduct this collection and to
request your voluntary assistance. The Census Bureau is required by Title 13, U.S.C., Section
9 to keep your information confidential and can use your responses only to produce
statistics. The Census Bureau is not permitted to publicly release your responses in a way that
could identify your business, organization, or institution. Per the Federal Cybersecurity Enhancement Act of 2015, your data are protected from cybersecurity risks through screening of the
systems that transmit your data.
