Note: The instruments included in this OMB package have been revised and incorporate

the pilot test results. The question numbers correspond with the instruments pilot tested.

Time to complete:

  • Less than 10 minutes

  • 5 minutes - but much left blank - it should really take more like 15 to be completed

Projected time to complete the survey is 10 minutes.

General Comments: The initial questions make sense only if the audience to be surveyed are all people who received MDRBD training (i.e., q2 assumes participation, q3 does not).

Have added a “Did not participate in trainings” response option to Question 2.

Format: I am assuming it will be web based.

Yes, the survey will be Web-based.

Format: There appears to be some redundancy (q6 and q10 for ex) - or at least overlap.

Question 6 addresses factors that precipitated consultation whereas Question 10 addresses the outcome of consultation. To address this feedback, we have changed Q6 to “What patient issue(s) prompted you to contact….”

Question 1: Can change to "what proportion/percentage of your practice is spent managing treatment..."

The suggested change has not been made because responses in the form of proportion/percentage will not provide information sought by this question.

Question 2: Give examples for all the options like there is for the ECHO example.

The suggested change has not been made due to risk of limiting options for the respondent.

Question 3: "How many of THESE trainings…"

The suggested wording change has been made.

Question 4: Add "Right now, I feel..."

The suggested wording change has been made.

Question 5: "What was the most common request for assistance you contacted the ....in the last 12 months?"

The suggested change has not been made to avoid changing the meaning of the question.

Question 6: "Select all the topical areas you contacted MDRBD seeking assistance with in the past 12 months?"

The suggested change has not been made to avoid changing the meaning of the question. However, have changed “patient concern(s)” to “patient issue(s)”.

Question 7: "...how often did you interact with...via the following modes of communication?"

The suggested wording change has not been made. How often health care providers interact through the modes of communication will be addressed through the ranking.

Question 8: "I MOST prefer...."

The suggested wording change has been made.

Question 9: Is not clear if it's referring to ANYONE (people in my practice, etc.) or only those with the program - needs to be clarified.

Have added language to clarify that this is behavioral health specialists from the MDRBD program.

Question 10: "IN the last 12 months...." & need to specify all these for at least ONE patient (quality vs. quantity or frequency).

The suggested wording change has been made to add “In the last 12 months”. In addition, response options have been made plural to indicate that this is for one or more patients.

Question 11: "...has informed my clinical management of..." - ? specific? - NOT clear what it is referring to.

Question 12: "In the last 12 months,..."

Question 14: ".... program facilitates easier access to..."

The question has been changed to: “My interaction with the MDRBD program increased referrals to specialty behavioral health services for my pregnant and postpartum patients.”

Question 15: "in the last 12 months..." & is not clear what comparison it is getting at with "other than..." - please clarify.

The question has been revised to read: As a result of the MDRBD program, in the last 12 months, more of my pregnant and postpartum patients received community-based services (e.g., housing support, parenting support) to improve their behavioral health.

Question 16: "in the last 12 months, with assistance from the MDRBD program..."

The wording has been changed to “In the last 12 months, my interaction with the MDRBD program increased my pregnant and postpartum patients’ use of services in the community to support their behavioral health.”

Question 17: "lasting changes"? - not clear, be more specific.

Have changed to “What clinical practices have you adopted…”

Question 18: Add "overall,..."

The suggested wording change has been made.

Question 19: "Currently what additional assistance do you still need to..."

The suggested wording change has been made.

Question 20: You or "your practice"?

The wording has been changed to help clarify the question as follows: “What behavioral health screening tool(s) do you administer, interpret, or act upon?”

Question 21: You or your practice?

The wording has been changed to “What behavioral health treatments do you personally provide” because we want to know about providers’ clinical actions.

Question 22: ..."I am NOW more comfortable..."

The suggested wording change has been made.

Question 23: This is not clear, also if you only treat pregnant/postpartum pts, this doesn't make any sense.

Have clarified this question as follows: “I am as comfortable assessing and treating pregnant and postpartum patients with behavioral health conditions as I am assessing and treating other comorbid conditions in pregnant and postpartum patients.”

Question 24: Add behavioral health provider?

The suggested change has been not made as this survey is intended for health care providers who may be accessing clinical consultation and care coordination support services from the network of behavioral health providers in the MDBRD program.

Question 25: Want to add if practice has trainees?

Trainees may be present in a number of these primary clinical practice sites so this has not been added.
