Attachment 3: Population-based Surveillance of Outcomes, Needs, and Well-being of Children and Adolescents with Congenital Heart Defects
Survey Sites: CHSTRONG-KIDS
CDC anticipates administering the CHSTRONG-KIDS survey at 3 sites, including the Metropolitan Atlanta Congenital Defects Program (MACDP). Applications for extramural funding of additional sites are currently under review. After awards have been made, CDC will provide the final list of sites and update the total estimated burden, if appropriate.
