Is the person an air crew member on official duty? (If yes, compliance check stops)
Is the air passenger a member of the US military on active orders?
Is the air passenger a US citizen, lawful permanent resident, non-US citizen traveling on immigrant
visa, refugee, parolee, asylee or follow-to-join family member?
If applicable, does the air passenger have proof of being fully vaccinated for COVID-19 per CDC’s
Order: Implementing Presidential Proclamation on Safe Resumption of Global Travel During the
COVID-19 Pandemic?
o If noncitizen nonimmigrant is not fully vaccinated, do they meet an exception to the proof of
full vaccination requirement?1
Does the air passenger have a negative COVID-19 test to show or proof of recent recovery from
COVID-19 that meets the requirements of CDC’s Amended Order: Requirement for Proof of Negative
COVID-19 Test Result or Recovery from COVID-19 for All Airline Passengers Arriving into the United
States?
o If not, do they have a valid humanitarian exemption?
Any other information needed to confirm the air passenger meets compliance of the Orders
