In this section, you will be asked about several things that someone from work might have
done to you that were upsetting or offensive, and that happened in the PAST YEAR.
When the question says “someone from work,” please include any person you have contact
with as part of your military duties. “Someone from work” could be a supervisor, someone
above or below you in rank, or a civilian employee/contractor. They could be in your unit or
in other units.
These things may have occurred on-duty or off-duty, on-base or off-base. Please include
them as long as the person who did them to you was someone from your military work. This
could be involving someone of any gender, who is Military Personnel (Active Duty or
National Guard/Reserve) who are On- or Off- duty, and On- or Off-installation or ship; or
