NQDW Services Survey Data

• Quarter 3/Quarter 4, 2019 (July 1 – December 31)*

*The form for submitting your 2019 Q3/Q4 Services Data is attached to this email.

You may send the de-identified Individual-Level Intake data through NORC’s SFTP site. Please contact [email protected] if you need assistance logging into NORC’s SFTP.

As a reminder, the CDC Cooperative Agreement for tobacco cessation Quitline services requires participation by all funded entities in the National Quitline Data Warehouse as a condition of receipt of funding.
