Hello, I’m [name], and I’m calling on behalf of the National Institute for Occupational Safety and Health, commonly known as NIOSH. To better understand current safety practices, prevent future line-of-duty injuries and deaths, and improve how they distribute information to the United State Fire Service, NIOSH is conducting a short survey of U.S. fire departments. . Your department was selected to participate in the survey, and we are calling all departments that we could not reach by email.

Please know that CDC/NIOSH will treat data/information in a secure manner and will not disclose unless otherwise compelled by law.

We take your privacy very seriously. If information sent through government networks triggers a cyber threat indicator, the information may be intercepted and reviewed for cyber threats by computer network experts working for, or on behalf of, the government.

Would it be possible for me to send a link to the survey to someone in your department? I can also provide you with the link right now.

  • [If yes, can send to someone in department]

    • Thank you! Could you provide me with the best email address to send the link to?

    • I will send an email with the link as soon as we get off the phone. NIOSH is trying to get information from chief officers, company officers and firefighters, and they can all use the link to complete the survey. They will only be asked to indicate their rank, but all of the information will be kept completely confidential.
